    Hi,
    We are in WA state but employer is in MA. Insurance is Aetna. Our Dr. put in for histrelin implant through a specialty pharmacy and the insurance came back that it needs to go through our medical benefits and not our pharmacy benefits. The pharmacy said we need to talk to the Dr. office. The Dr. office (Polyclinic, Seattle) just messaged back through mychart that they’re going to put in for approval for an injectable alternative. However, Aetna’s gender affirming care policy (https://www.aetna.com/cpb/medical/data/500_599/0501.html) says that histrelin is approved. Anyone deal with this before?
