Primera coverage 14 year old FTM top surgery?

I live in WA state and have a 14 year old that has support from his full medical team to receive top surgery. Premera has denied the initial request. Does anyone have any suggestions on how to move forward in the appeal process? We are currently gathering more letters with a stronger reference for medical necessity to submit in our first appeal. Any thoughts, suggestions or experiences from others would be wonderful.
